Social Prescribing Link Worker – SAVS

SAVS are looking to recruit:

Social Prescribing Link Worker
37 hours per week (to include some evening & weekend working)
This post is funded until 31 March 2023
Salary: £22,500 per annum

Social Prescribing is a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services. This service is provided as part of a model of partnership between the voluntary, community, faith and social enterprise sector (VCFSE) and GPs. This post is funded by the NHSE through the local PCNs.

The role will be to:

―     Create, develop and maintain solid trusting relationships with GP practices and health professions and voluntary sector partners

―     Work with and take referrals from wide range of agencies

―     Provide support to individuals in a holistic manner

The successful candidate will have experience, a sound knowledge of the local area and range of voluntary, statutory and private sector organisations available to support local people.

Full clean driving licence essential.
